Welcome to the Archbishop Carroll Sports Medicine page. Our mission is to provide quality, evidence-based healthcare to your student-athlete through athletic injury prevention, rehabilitation and performance enhancement.  Below you will find attachments to important documents that your student-athlete will need to participation in athletics at our school.

 REQUIRED FORMS

Physical: All student-athletes are required to obtain Pre-Participation Physicals after June 1 preceding the start of school.  The physical form must be completely filled out to include emergency contact, parent signature, physician signature and date of physical. The form will not be accepted in any information is incomplete.
