1.
Smoking is prohibited in all areas of the facility.
2.
Children accompanying adults must be supervised at all times.
3.
All food and drink shall be confined to Carroll Café.
4.
Certificates of General Liability Insurance may be required for functions held at ACHS. Host group or individual will be notified upon submission of a completed facility use request form. Insurance is mandatory for all events at which alcohol will be served.
5.
ACHS reserves the right to decline rental of the facility, rescind a rental contract, or require a cash deposit as prerequisite to rental.
6.
The Lessee agrees to assume and be responsible for any expense(s) incurred for damage(s) to the
premises or property resulting from Lessee’s use.
7.
ACHS does not accept responsibility for loss or theft of articles belonging to Lessee or any member of this party, or injury or accident to any said person on the ground of or in the facility, and is absolved by Lessee of all liability, which may result thereof.
8.
Additional attendants, i.e., parking attendants, cleaning personnel, security supervision, etc., may
be required as a condition of approval, the cost of which shall be Lessee’s responsibility.
9.
Outdoor activities cease at 10:00PM as required by local zoning laws and in consideration of our
neighbors.
10.
Taping, mounting or displaying items on any painted surface is not allowed.
11.
Any items/supplies brought into the facility or placed on the property must be removed within 24
hours of the conclusion of the Lessee’s event. Items remaining after 24 hours may be disposed of
as abandoned property.
12.
An ACHS staff member will be on site and available to assist you, or your designated
representative, throughout your rental time unless other arrangements have been made with the
Principal/CEO prior to your scheduled event.
Gym use by outside groups will not be available
until 8:00PM during the school week.
13.
On the days of an emergency school
closing or on the days school sponsored practices are
cancelled, all events scheduled within the buildings by outside groups are cancel led.
14.
During Christmas or Spring Breaks, it will be determined by the
Principal/CEO or designee
whether the facilities will be available to use based upon the
school maintenance schedules.
15.
When using a classroom, all furniture shall be returned to its prope
r place. Classroom supplies and materials should not be used at any time.
16.
Groups utilizing school facilities/properties are responsible for supervision/enforcement of
parking in “designated areas” only. “Designated areas” are described as parking lots or o
n the street. Parking on/in the following areas is strictly prohibited: a.) Grass b.) Posted no parking
areas c.) Emergency vehicle access roads/fire lanes d.)School entrances/exits/receiving areas.
In requesting the use of Archbishop Carroll High School (ACHS), I hereby acknowledge that I have read
and understand the above terms and conditions, that the information provided by myself is true and
accurate, and that I accept full and complete responsibility for the conduct and safety of the group and any
and all damages that may result as a consequence of the rental.
